Course description:

BMSC and Palladium are pleased to present this joint course.

Our innovative, interactive course incorporates Safety-I and Safety-II thinking into a practical approach to incident investigation.

Delivered by experienced educators with practical expertise in the use of human factors and investigation techniques. Transforming care by training hundreds of multi-professional healthcare staff, in the art of effective error investigation and action plan implementation.

Workshops are delivered in established education venues to provide the optimum training experience.  Workshops provide ‘hands on’ practice in human factors analysis of events through combining advocacy inquiry debriefing skills with tested investigation techniques – all with the aim of discovery and learning from patient safety incidents.

How is our course different?

Our 3-day course teaches an approach that facilitates the analysis of human performance and systems combining a range of investigative tools to investigate patient safety incidents. Teaching will include; structured approach to interview; debriefing techniques to elicit clinician mental models and incorporation of human factors analysis during incident analysis.

In addition the course arms candidates with practical team leadership skills to prepare for and run an incident review meeting and implement changes to improve patient safety through quality improvement.

  • Clinical or managerial staff with responsibility for reviewing completed incident investigation reports and their action plans

  • Staff with an interest in understanding how human factors can improve the quality of investigations and learning
